Tees, Esk and Wear Valleys NHS Foundation Trust (TEWV) - who provide mental health and learning disability services for people of all ages in County Durham and Darlington, Teesside, North Yorkshire, York, and Selby - is inviting feedback from service users, carers and partners on its updated Care Planning Policy. This update follows an NHS directive aimed at shifting away from generic care coordination for people in the community to a more personalised, intervention-based approach.
The revised policy is designed to provide individuals with more choice and flexibility in their care. It focuses on tailoring care to meet each person’s specific health, care and life needs while taking into account the circumstances of their carers and family members. The approach places service users, carers and families at the heart of the care planning process promoting collaboration between the NHS and wider partners, such as the police, local councils and the voluntary, community and social enterprise sector.
Skipton and Ripon MP, Rt Hon Sir Julian Smith KCB CBE, is urging those with experience of TEWV’s services, whether in a personal or professional capacity, to get their views into the consultation.
